【摘要】目的 探討重癥急性胰腺炎(SAP)中IL-10及IL-18的表達及其與SAP肺損傷的關系。
方法 將48只SD大鼠按隨機數(shù)字表法均分為對照組和SAP組,動態(tài)定量測定肺濕重系數(shù)、腹水量、血清淀粉酶、血清IL-10及IL-18的變化和半定量RT-PCR檢測肺組織中IL-10 mRNA及IL-18 mRNA的表達,并在光鏡下觀察胰腺及肺組織病理學改變。
結(jié)果 各時相SAP組肺濕重系數(shù)、腹水量、血清淀粉酶、血清IL-10及IL-18水平和肺組織中IL-10 mRNA及IL-18 mRNA的表達與對照組相比均明顯增高(P<0.05,P<0.01)。SAP組大鼠血清中IL-18水平和肺組織IL-18 mRNA表達水平均與肺濕重系數(shù)(r=0.68,P<0.01; r=0.72,P<0.01)及肺損傷病理評分(r=0.74,P<0.01; r=0.79,P<0.01)呈正相關,而血清中IL-10水平和肺組織IL-10 mRNA表達水平均與肺濕重系數(shù)(r=-0.62, P<0.01; r=-0.69, P<0.01)及肺損傷病理評分(r=-0.66,P<0.01; r=-0.60,P<0.01)呈負相關。
結(jié)論 IL-18參與了SAP并發(fā)肺損傷的病理過程,而IL-10對肺損傷有保護作用。
