目的 觀察無(wú)水乙醇腹腔神經(jīng)叢阻滯術(shù)對(duì)晚期胰腺癌頑固性疼痛的鎮(zhèn)痛效果、并發(fā)癥發(fā)生及患者生存質(zhì)量的影響。方法 選取2001年1月至2005年9月61例晚期胰腺癌伴頑固性疼痛及消化道梗阻無(wú)法行根治手術(shù)的患者，在行姑息性手術(shù)的同時(shí)，行腹腔神經(jīng)叢無(wú)水乙醇阻滯治療，觀察術(shù)后3個(gè)月內(nèi)疼痛緩解、生存質(zhì)量評(píng)分（KPS）的變化及不良反應(yīng)。結(jié)果 治療后1周內(nèi)全部患者頑固性疼痛癥狀明顯緩解 (P＜0.05)， KPS評(píng)分顯著提高 (P＜0.05)。隨診3個(gè)月，術(shù)后86.5% (45/52) 患者無(wú)痛或僅輕度疼痛，治療過(guò)程中和治療后未發(fā)生嚴(yán)重并發(fā)癥。結(jié)論 無(wú)水乙醇腹腔神經(jīng)叢阻滯治療晚期胰腺癌頑固性疼痛能較好地緩解其癥狀，改善患者生存質(zhì)量，近期療效確切，安全性高。
