目的探討應(yīng)用低位腹主動脈內(nèi)球囊阻斷技術(shù)在骨盆或骶骨腫瘤切除術(shù)中的臨床應(yīng)用價值。方法 2006年4月至2010年4月期間我院應(yīng)用低位腹主動脈內(nèi)球囊阻斷技術(shù)控制24例骨盆和骶骨腫瘤手術(shù)過程中的出血。術(shù)中先用球囊導(dǎo)管阻斷低位腹主動脈，每次阻斷腹主動脈60 min，若超過60 min間隔10～15 min后再阻斷，然后再行腫瘤切除。結(jié)果本組患者平均手術(shù)時間為153 min (40~245 min); 阻斷低位腹主動脈后，手術(shù)中平均出血為310 ml (200~650 ml)，手術(shù)野干凈，解剖清晰，腫瘤周圍的血管、神經(jīng)以及盆腔器官明確，腫瘤邊界容易確定，腫瘤被完整切除，腫瘤的術(shù)野污染小。手術(shù)全過程中，患者血壓穩(wěn)定，術(shù)后患者腎功能及盆腔器官均正常。1例患者發(fā)生穿刺側(cè)髂動脈血栓形成，通過Fogarty取栓導(dǎo)管取出血栓后下肢血供恢復(fù)，術(shù)后無缺血癥狀，其余患者雙下肢均正常，無并發(fā)癥發(fā)生。結(jié)論應(yīng)用低位腹主動脈內(nèi)球囊阻斷技術(shù)切除骨盆和骶骨腫瘤可有效減少術(shù)中出血，提高手術(shù)的安全性和完整切除率，是一項具有實用價值的臨床技術(shù)。
