目的 探討術中超聲引導下精準Tru-cut活檢在胰頭實性腫塊病理學診斷中的價值。
方法 對徐州醫(yī)學院附屬宿遷醫(yī)院肝膽胰脾外科2010年8月至2011年8月期間收治的胰頭實性腫塊患者行術中超聲引導下精準Tru-cut活檢,共28例患者,男20例,女8例,年齡 34~78歲,平均 64歲; 腫塊最大徑≤1.5cm者5例(0.8~1.5cm ,平均1.2cm,S組),1.5cm<腫塊最大徑≤3.0cm者7例(1.6~3.0cm,平均2.5cm,M組),腫塊最大徑>3.0cm者16例(3.2~6.8cm ,平均4.8cm,L組),每一腫塊活檢3針,每一條組織條均行快速冰凍檢查,其結果與術后大標本石蠟切片病理結果分別比較。
結果 以患者為觀察單位,所有病例診斷符合率為100%,無假陽性及假陰性。以單個組織條病理結果為觀察單位,僅1條符合(1/3)、僅2條符合(2/3)和3條全符合(3/3)者分別為:S組是3例 (3/5)、2例 (2/5)、0 例(0/5);M組是0例 (0/7)、5例 (5/7)、2例 (2/7);L組是0例 (0/16)、4例(4/16)、12例 (12/16); 在小病灶組(S組+M組)中的單條組織標本的假陰性率高于L組(χ2=9.833,P=0.002)。病灶小易出現單次穿刺假陰性結果。
結論 術中超聲引導下精準Tru-cut活檢能夠準確確定胰頭腫塊的病理學診斷,安全有效,可為手術決策提供可靠依據。
