• 汕頭大學(xué)醫(yī)學(xué)院第一附屬醫(yī)院普外科（廣東汕頭 515041）;

目的 　評(píng)價(jià)腹腔鏡經(jīng)腹腔腹膜前（TAPP）和完全腹膜外（TEP）兩種手術(shù)入路在原發(fā)性腹股溝疝患者中的治療效果。
方法 　制定嚴(yán)格的納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)，收集腹腔鏡TAPP與TEP原發(fā)性腹股溝疝修補(bǔ)術(shù)臨床療效比較的隨機(jī)對(duì)照試驗(yàn)，按照Cochrane協(xié)作網(wǎng)推薦的方法對(duì)納入研究進(jìn)行系統(tǒng)分析。
結(jié)果 　7篇隨機(jī)對(duì)照試驗(yàn)（552例患者）納入分析，TAPP組302例，TEP組250例。TAPP組與TEP組相比較，2組在手術(shù)時(shí)間 〔WMD=8.49，95% CI （-1.37，18.35），P=0.09〕、術(shù)后住院時(shí)間〔SMD=0.11，95% CI （-0.12，0.34），P=0.34〕、術(shù)后復(fù)發(fā)率 〔OR=1.37，95% CI （0.36，5.20），P=0.64〕、并發(fā)癥（切口感染、尿潴留）發(fā)生率 〔切口感染：OR=4.27，95% CI （0.85，21.57），P=0.08；尿潴留：OR=0.61，95% CI （0.21，1.78），P=0.36〕方面差異均無(wú)統(tǒng)計(jì)學(xué)意義，但2組在術(shù)后24h疼痛方面，TEP組術(shù)后疼痛更輕〔WMD=0.72，95% CI （0.58，0.87），P＜0.000 01〕。
結(jié)論 　目前證據(jù)顯示，TEP術(shù)后24h疼痛程度比TAPP術(shù)更輕；手術(shù)時(shí)間、術(shù)后住院時(shí)間、術(shù)后復(fù)發(fā)及并發(fā)癥方面2組差異無(wú)統(tǒng)計(jì)學(xué)意義，但仍需更多高質(zhì)量的大樣本隨機(jī)對(duì)照試驗(yàn)進(jìn)一步證實(shí)。
