目的  探討經(jīng)胸乳入路行腔鏡下甲狀腺切除的可行性與安全性并總結(jié)手術(shù)技巧。
方法  對(duì)我院2010年8月至2012年8月期間的40例經(jīng)胸乳入路行腔鏡下甲狀腺切除患者的臨床資料進(jìn)行回顧性分析。
結(jié)果  40例患者均經(jīng)胸乳入路完成腔鏡下甲狀腺切除,無中轉(zhuǎn)開放病例,無大出血、高碳酸血癥、嚴(yán)重皮下氣腫及胸前皮膚壞死的病例。未發(fā)生永久性喉返神經(jīng)損傷及永久性甲狀旁腺功能低下。1例患者術(shù)后2d出現(xiàn)聲音嘶啞,對(duì)癥處理后恢復(fù)正常。1例患者術(shù)后2d出現(xiàn)四肢末端麻木,予以靜脈及口服補(bǔ)鈣治療3 d后癥狀緩解。1例患者術(shù)后有頸胸部皮膚發(fā)緊不適感,1個(gè)月后自行緩解。手術(shù)時(shí)間(102±28.4) min (55~182min),術(shù)中出血量為(46±16.6) mL (30~106mL),術(shù)后引流管留置時(shí)間(4±2.2) d (2~7d),術(shù)后住院時(shí)間(4±1.1) d (3~8d)。術(shù)后平均隨訪13個(gè)月,無低鈣、低甲狀旁腺激素的病例,無聲音嘶啞及局部甲狀腺腫復(fù)發(fā)的病例;2例患者出現(xiàn)甲狀腺功能低下,給予調(diào)整口服甲狀腺素劑量后恢復(fù)正常。所有患者對(duì)美容效果滿意。
結(jié)論  經(jīng)胸乳入路行腔鏡下甲狀腺切除是安全、可行的,美容效果滿意?!癥”形的穿刺隧道、“上黃中白下紅”的胸前間隙分離和倒梯形的頸部間隙標(biāo)志是建立操作空間的關(guān)鍵,而充分的暴露、循序漸進(jìn)的操作順序和尋準(zhǔn)間隙銳鈍結(jié)合的分離是腔鏡下腺體切除的手術(shù)要點(diǎn)。
