目的總結(jié)鋼板內(nèi)固定聯(lián)合人工全髖關(guān)節(jié)置換治療老年移位性髖臼骨折的臨床療效。 方法2004年5月－2009年5月，采用鋼板內(nèi)固定聯(lián)合人工全髖關(guān)節(jié)置換治療32例老年單髖移位性髖臼骨折。男27例，女5例；年齡60～83歲，平均67歲。致傷原因：交通事故傷12例，跌傷14例，高處墜落傷6例。受傷至手術(shù)時(shí)間為3～16 d，平均7 d。髖臼骨折根據(jù)Letournel 分型標(biāo)準(zhǔn)：前柱骨折9例，前壁骨折8例，T型骨折12例，前柱合并后半橫型骨折1例，后壁合并橫型骨折2例。合并骨質(zhì)疏松12例。 結(jié)果術(shù)后切口均Ⅰ期愈合，無(wú)相關(guān)并發(fā)癥發(fā)生。患者均獲隨訪，隨訪時(shí)間25～89個(gè)月，平均42.2個(gè)月。術(shù)后2～4個(gè)月患者均能完全負(fù)重行走，平均3.5個(gè)月。末次隨訪時(shí)髖關(guān)節(jié)Harris 評(píng)分為78～91分，平均85分。6例發(fā)生異位骨化，1例發(fā)生骨溶解；其余患者隨訪期間均無(wú)假體松動(dòng)和下沉。 結(jié)論鋼板內(nèi)固定聯(lián)合人工全髖關(guān)節(jié)置換適用于治療合并創(chuàng)傷性關(guān)節(jié)炎、股骨頭骨折、股骨頸骨折、軟骨損傷的老年移位性髖臼骨折，臨床療效滿意。
