目的 總結(jié)關(guān)節(jié)鏡下治療髖關(guān)節(jié)滑膜軟骨瘤的方法及臨床療效。 方法2009年7月-2011年6月,對(duì)15例髖關(guān)節(jié)滑膜軟骨瘤于關(guān)節(jié)鏡下行滑膜清理和游離體摘除術(shù)。男11例,女4例;年齡21~45歲,平均33.1歲。左髖6例,右髖9例。病程12~43個(gè)月,平均23個(gè)月。主要臨床癥狀為患側(cè)髖關(guān)節(jié)活動(dòng)受限、脹痛。疼痛視覺模擬評(píng)分(VAS)為(5.8 ± 1.1)分,髖關(guān)節(jié)活動(dòng)度為(149.8 ± 27.5)°,Harris評(píng)分為(54.5 ± 13.3)分。 結(jié)果術(shù)后切口均Ⅰ期愈合?;颊呔@隨訪,隨訪時(shí)間6個(gè)月~2年,平均17.4個(gè)月。末次隨訪時(shí)VAS評(píng)分為(2.0 ± 1.2)分,髖關(guān)節(jié)活動(dòng)度為(258.3 ± 35.4)°,Harris評(píng)分為(93.0 ± 18.7)分,以上指標(biāo)均較術(shù)前顯著改善,差異有統(tǒng)計(jì)學(xué)意義(P  lt; 0.05)。隨訪期間MRI檢查示均無復(fù)發(fā)。 結(jié)論關(guān)節(jié)鏡下治療髖關(guān)節(jié)滑膜軟骨瘤創(chuàng)傷小,術(shù)后恢復(fù)快,可最大程度恢復(fù)髖關(guān)節(jié)功能和活動(dòng)度。
