【摘 要】 目的 探討股靜脈作為血管移植材料的可行性與安全性。 方法 取60具成人尸體共114側(cè)下肢標(biāo)本,解剖觀察股靜脈、股深靜脈、腘靜脈及靜脈交通支,測量股深靜脈匯入股靜脈處至收肌腱裂孔下緣的股靜脈段長度,即股靜脈可切取的解剖長度,以及靜脈壓扁外徑。分析2010年3月-2011年5月收治的120例下肢股靜脈段血栓形成患者CT靜脈造影(computed tomography venography,CTV)檢查資料,觀察其下肢靜脈回流通路。 結(jié)果 男性尸體身高平均158.3 cm,股靜脈可切取長度為(18.8 ± 2.3)cm,相對長度為0.118 ± 0.013,靜脈壓扁外徑為(15.8 ± 0.8)mm;女性尸體分別為149.2 cm、(15.1 ± 1.5)cm、0.101 ± 0.010、(14.0 ± 1.1)mm。男女股靜脈可切取長度比較,差異有統(tǒng)計學(xué)意義(t=6.354, P=0.000);靜脈壓扁外徑比較差異有統(tǒng)計學(xué)意義(t=5.555,P=0.000)。股靜脈可切取長度與身高成正相關(guān)(r=0.964,P=0.000),股靜脈壓扁外徑與身高成正相關(guān)(r=0.382,P=0.003)。解剖觀察見16側(cè)(14.0%)肢體存在雙股靜脈變異支,48側(cè)(42.1%)肢體存在1支股-腘靜脈交通支,38側(cè)(33.3%)肢體存在1支股深-腘靜脈交通支。CTV檢查示,下肢股靜脈血栓形成以后,大隱靜脈及股-腘或股深-腘靜脈交通支可代償股靜脈。 結(jié)論 大隱靜脈和股-腘靜脈交通支或股深-腘靜脈交通支的存在,保證了切取股靜脈作為血管移植材料的可靠性及安全性。
