目的 分析行非體外循環(huán)冠狀動脈旁路移植術(shù)(off-pump coronary artery bypass grafting,OPCAB)的高危患者術(shù)前應(yīng)用主動脈內(nèi)球囊反搏(intraaortic balloon pump,IABP)的治療效果,總結(jié)IABP的應(yīng)用及撤機指征?!》椒ā』仡櫺苑治鲎?008年1月至2011年7月復旦大學附屬中山醫(yī)院102例高危冠心病患者[IABP組,男71例,女31例;年齡(63.0±8.2)歲]在OPCAB術(shù)前置入IABP治療的臨床資料,另外選擇100例未于術(shù)前置入IABP的患者作為對照[對照組,男55例,女45例;年齡(64.1±9.5)歲] 。監(jiān)測兩組患者手術(shù)后有創(chuàng)動脈收縮壓(SABP)、有創(chuàng)平均動脈壓(MABP)、機械輔助通氣時間、住ICU時間、并發(fā)癥發(fā)生情況、IABP輔助時間、圍術(shù)期死亡等。術(shù)后3個月復查心臟超聲心動圖檢測左心室射血分數(shù)(LVEF)?!〗Y(jié)果 IABP組患者術(shù)后SABP[(95.3±12.2) mm Hg vs. (80.1±11.7) mm Hg;t=8.440,P=0.000]、MABP?。郏?8.9±13.5) mm Hg vs. (52.3±15.1) mm Hg; t=12.410,P=0.000]明顯高于對照組;機械輔助通氣時間、住ICU時間、正性肌力藥物輔助時間較對照組短;室性心律失常、低心排血量、圍術(shù)期心肌梗死和圍術(shù)期腎功能不全血液透析的發(fā)生較對照組少;圍術(shù)期死亡率低于對照組[5.9% (6/102) vs. 17.0% (17/100),χ2=6.180,P=0.020] 。IABP組隨訪96例,對照組隨訪83例,隨訪時間均為3個月。術(shù)后3個月時心臟超聲心動圖提示IABP組LVEF顯著高于對照組(45.3%±12.0% vs. 39.1%±8.2%,t=3.950,P=0.000)?!〗Y(jié)論 對具有高危因素的OPCAB患者術(shù)前預(yù)防性置入IABP、且把握好撤機時機,可降低手術(shù)風險,明顯提高手術(shù)效果,加快患者術(shù)后恢復,改善心功能,減少并發(fā)癥的發(fā)生,降低圍術(shù)期病死率。
