近年來(lái), 小潮氣量、呼氣末正壓( PEEP) 通氣及肺復(fù)張的肺保護(hù)性通氣策略已普遍被人們接受, 并越來(lái)越多的應(yīng)用于 ALI/ARDS 患者。但由此造成的呼吸機(jī)相關(guān)性肺損傷 ( VALI) , 及其對(duì)循環(huán)系統(tǒng)的不良反應(yīng)也逐步引起人們的重視, 如何選擇最佳PEEP 成為關(guān)注的熱點(diǎn)。Suter 等[ 1] 于 1975 年首先提出最佳PEEP( optimal PEEP) 的概念, 認(rèn)為既能改善氧合, 又能減少VALI 的PEEP 為最佳PEEP。過(guò)去我們通常認(rèn)為在FiO2 ≤ 60% 條件下, 使PaO2 ≥ 60 mm Hg ( 1 mmHg = 0. 133 kPa) 的最低PEEP 為最佳PEEP, 另有學(xué)者利用氧代謝選擇最佳PEEP, 認(rèn)為能達(dá)到最大氧輸送 ( maximal oxygen transport) 的最低PEEP, 才為最佳PEEP[ 2] 。許多學(xué)者認(rèn)為壓力-容積曲線(xiàn)( P-V 曲線(xiàn)) 吸氣支的吸氣支下拐點(diǎn)( lower inflection point, LIP) 為肺泡復(fù)張的結(jié)束, 將PEEP 設(shè)置在LIP + 2 cmH2O( 1 cmH2O= 0. 098 kPa) 附近可以避免肺泡塌陷。近年來(lái)隨著大量的臨床和動(dòng)物實(shí)驗(yàn)的開(kāi)展, 舊的理論逐漸被推翻, 出現(xiàn)了更多尋找最佳PEEP 的方法, 本文就ALI/ARDS 患者機(jī)械通氣時(shí)PEEP 選擇方法的研究進(jìn)展作一綜述。
