目的  研究小氣道功能與氣道敏感性和反應性的關系,探討氣道高反應性(BHR)變異的影響因素。方法  采用前瞻性研究,收集北京大學第三醫(yī)院2005年1月~2006年4月期間支氣管激發(fā)試驗(BPT)陽性疑診支氣管哮喘的患者,氣道敏感性以引起第1秒用力呼氣容積(FEV1)下降?20%的乙酰甲膽堿(Mch)累積量表示(PD20);氣道反應性以Mch劑量反應曲線斜率(DRS)表示,以FEV1下降率除以Mch累積量。以log10DRS作為因變量,以在50%肺活量位時的最大呼氣流速(Vmax50%)和在25%肺活量位時的最大呼氣流速(Vmax25%)反映小氣道功能,建立小氣道功能指標與log10DRS之間的簡單線性回歸模型。以log10DRS作為因變量,根據(jù)年齡、身高、FEV1%pred建立多元回歸模型,得出決定系數(shù)(r2)。將小氣道功能指標加入此模型,重新計算r2,二者差異代表小氣道功能指標在氣道反應性中的作用。分別對不同性別和年齡組進行分析。結果  共入選184例患者,男70例,女114例。PD20越低,Vmax50%和Vmax25%越低,反之亦然。PD20與log10DRS顯著負相關(r=-0.874,P lt;0.01)。簡單線性回歸分析發(fā)現(xiàn),F(xiàn)EV1%、Vmax50%、Vmax25%分別與log10DNS顯著相關(決定系數(shù)r2分別為0.062,0.097和0.085,P均 lt;0.01)。與年齡、身高、FEV1%pred進行多元線性回歸分析,Vmax50%、Vmax25%引起氣道反應性的變異程度分別占3.9%和2.6%。不論男性還是女性患者,Vmax50%與log10DNS顯著相關,并且男性r2高于女性。年齡?25歲者女性Vmax50%、Vmax25%與log10DNS顯著相關,r2高于男性。而年齡在25~45歲患者男性Vmax50%、Vmax25%與log10DNS顯著相關,r2高于女性。當年齡 gt;45歲時男性和女性患者Vmax50%、Vmax25%與log10DNS均無明顯相關性。結論  小氣道功能下降導致氣道敏感性和反應性增加。
