用犬制作膽道梗阻模型,觀察梗阻后不同時相犬十二指腸和回盲部需氧菌的變化,并對其腸壁進(jìn)行組織學(xué)與超微結(jié)構(gòu)觀察,以探討膽道梗阻對小腸菌群的影響和腸道菌群在膽道感染等內(nèi)源性感染中的作用。結(jié)果顯示:隨犬膽道梗阻時間的延長,小腸菌群發(fā)生不同程度的變化,表現(xiàn)為細(xì)菌的輕度增殖,定殖菌種數(shù)增多,下段小腸細(xì)菌(主要是大腸桿菌)移位到十二指腸,使十二指腸內(nèi)總需氧菌和大腸桿菌的定殖率明顯增高;回腸粘膜充血水腫、炎細(xì)胞浸潤、上皮微絨毛減少。破環(huán)等病理改變。由此表明,膽道梗阻時腸菌失調(diào)和腸粘膜屏障的破環(huán)可能與膽道梗阻時膽道感染發(fā)生率高有著密切關(guān)系。因此臨床上在及時解除膽道梗阻的同時,還應(yīng)注意調(diào)整膽道梗阻患者的腸道菌群平衡,阻止下消化道細(xì)菌上行定植,以防止腸源性感染的發(fā)生。
