通過(guò)在蛙皮縮膽囊素(caerulein,Cn)誘導(dǎo)的輕癥胰腺炎時(shí)用阿片類(lèi)止瀉劑(lopemin)抑制腸運(yùn)動(dòng)和在脫氧膽酸胰管注射的重癥胰腺炎時(shí)用大黃促進(jìn)腸運(yùn)動(dòng)的兩個(gè)系列鼠實(shí)驗(yàn)為基礎(chǔ),探討了腸道運(yùn)動(dòng)力學(xué)改變對(duì)急性胰腺炎的病程和預(yù)后的影響。結(jié)果顯示: 抑制運(yùn)動(dòng)將增加胰腺炎的繼發(fā)感染和內(nèi)毒素血癥,反之促進(jìn)腸運(yùn)動(dòng)則減少細(xì)菌移位和內(nèi)毒素血癥,從而降低實(shí)驗(yàn)大鼠的死亡率。提示: 腸動(dòng)力學(xué)改變與急性胰腺炎的病程及預(yù)后有密切關(guān)系,通過(guò)促進(jìn)腸道運(yùn)動(dòng)可減少急性胰腺炎的繼發(fā)感染和多器官衰竭,從而改善胰腺炎的預(yù)后。
