目的 探討手術(shù)操作是否會(huì)導(dǎo)致肝癌細(xì)胞的血行播散。方法 選擇14例無(wú)遠(yuǎn)處轉(zhuǎn)移并行肝癌切除術(shù)的肝癌患者，提取患者術(shù)前和術(shù)后外周血標(biāo)本，于術(shù)中腫瘤切除后提取門(mén)靜脈和肝靜脈血標(biāo)本。采用巢式RT-PCR方法檢測(cè)血中AFP-mRNA的表達(dá)，觀(guān)察手術(shù)前后血液中AFP-mRNA表達(dá)陽(yáng)性率及表達(dá)水平的變化。 結(jié)果 術(shù)前外周血、手術(shù)切除腫瘤后門(mén)靜脈血和術(shù)后外周血中AFPmRNA表達(dá)陽(yáng)性率分別為42.9%，42.9%和35.7%，術(shù)中切除腫瘤后肝靜脈血中的表達(dá)陽(yáng)性率為57.1%，雖高于前三者，但無(wú)統(tǒng)計(jì)學(xué)差異，但AFP-mRNA表達(dá)水平明顯高于術(shù)前及術(shù)后外周血和術(shù)中門(mén)靜脈血（P＜0.05）。結(jié)論 手術(shù)中有包括肝癌細(xì)胞在內(nèi)的肝臟來(lái)源的細(xì)胞經(jīng)肝靜脈進(jìn)入血循環(huán)。
