目的提高晚期膽囊癌的生存率及治療效果。方法對1990年1月至2001年6月我科手術(shù)治療的80例晚期膽囊癌患者的資料進(jìn)行回顧性分析，其中71例伴有阻塞性黃疸，15例捫及腹部包塊。對已浸潤周圍器官及膽管但尚無肝臟廣泛轉(zhuǎn)移或遠(yuǎn)處轉(zhuǎn)移的39例晚期膽囊癌進(jìn)行了擴(kuò)大根治術(shù)(其中11例合并胰十二指腸切除術(shù))。結(jié)果該39例術(shù)后存活8～37個月(平均存活18.1個月)。1、2、3年生存率分別為43.6%、20.5%及5.1%。對另41例已有肝臟轉(zhuǎn)移或腹膜種植轉(zhuǎn)移的晚期膽囊癌僅行姑息性手術(shù)，術(shù)后隨訪除1例存活19個月外，其余均于1年內(nèi)死亡。結(jié)論對晚期膽囊癌的治療應(yīng)盡可能地行擴(kuò)大根治術(shù)。
