目的觀察重癥急性膽管炎(ACST)患者術(shù)后外周血單核細(xì)胞(PBMC)中核因子kB(NFkB)活性的變化及其與膽道感染嚴(yán)重程度和預(yù)后的關(guān)系。 方法20例ACST患者根據(jù)其預(yù)后情況分為生存組(14例)和死亡組(6例),同時(shí)選擇胃潰瘍行大部切除術(shù)或疝修補(bǔ)術(shù)等非炎癥疾病患者10例作為對照組,分別于術(shù)后24 h取外周血, 用密度梯度離心法分離PBMC,提取核蛋白, 并用凝膠電遷移改變分析法(EMSA)測定PBMC中NFkB活性的變化, 所得結(jié)果用圖像分析儀分析并以相對光密度(ROD)表示。用酶聯(lián)免疫法(ELISA)測定血漿中TNFα、IL6和IL10含量的變化。結(jié)果ACST死亡組NFkB活性最高(5.02±1.03),生存組次之(2.98±0.51),對照組最低(1.02±0.34),各組間比較差異有顯著性意義(P<0.05)。ACST死亡組血漿TNFα和IL6含量明顯增高,分別為(496.28±52.35) ng/L和(578.13±67.72) ng/L,生存組分別為(284.47±39.41) ng/L和(318.67±34.92) ng/L,對照組分別為(89.43±10.39) ng/L和(101.27±13.47) ng/L, 各組間比較差異有顯著性意義(P<0.05)。 ACST死亡組和生存組IL10分別為(378.42±41.02) ng/L和(384.75±39.24) ng/L,均明顯高于對照組的(69.36±11.05) ng/L(P<0.05),死亡組與生存組之間差異無顯著性意義(P gt;0.05)。結(jié)論ACST時(shí)患者PBMC中NFkB的活性顯著增高,增高的NFkB能較好地預(yù)測膽道感染的嚴(yán)重程度及其預(yù)后。
