实时三维导航技术应用于先天性脊柱侧凸矫形的近期疗效_《中国修复重建外科杂志》

目的探讨术中应用实时三维导航技术辅助椎弓根螺钉植入矫治先天性脊柱侧凸（congenital scoliosis，CS）的近期疗效。方法2010年7月－2011年12月，收治CS患儿26例，其中13例术中采用实时三维导航技术辅助椎弓根螺钉植入（导航组），13例采用传统X线机监视下徒手植入椎弓根螺钉（对照组）。两组患儿性别、年龄、半脊椎个数及位置、主弯Cobb角以及Risser征比较，差异均无统计学意义（P gt; 0.05），具有可比性。记录两组患儿手术时间、术中出血量、螺钉重植率以及并发症发生情况；术后通过CT采用Richter法评价椎弓根螺钉位置，摄全脊柱X线片评价矫形效果。结果患儿均顺利完成手术，无神经、血管损伤等并发症发生。两组患儿手术时间、术中出血量以及植钉部位比较，差异均无统计学意义（P gt; 0.05）；导航组术中螺钉重植率（5.2%）显著低于对照组（16.7%），差异有统计学意义（χ2=3.975，P=0.046）。导航组及对照组患儿均获随访，随访时间分别为6～24个月和6～23个月。末次随访时两组均未见脊柱失代偿及矫形效果丢失。根据Richter法评价，术后1周导航组螺钉位置获优52枚、良6枚，对照组获优51枚、良5枚、差4枚，比较差异有统计学意义（Z=—1.992，P=0.046）。两组术后1周及末次随访时主弯Cobb角均较术前显著改善，差异有统计学意义（P lt; 0.05）；术后1周与末次随访时比较差异无统计学意义（P gt; 0.05）。两组末次随访时Cobb角矫正率比较差异无统计学意义（t=0.055，P=0.957）。结论术中实时三维导航技术可有效提高CS术中椎弓根螺钉植入的精确性，降低螺钉重植率，近期疗效较好。
